There are certain things that are essential or useful for you to know as you start on your treatment journey to collect (and store) your eggs. Please watch this video attentively to ensure you are fully informed, or to find out what else you need to know as you begin your treatment with us.
Once you have finished the video, please let us know in the entry form beneath the video, so that you can progress to the next stage of your treatment. Please note; you cannot proceed to the next stage of your treatment until we receive confirmation that you have watched this video.

Please note: This information video refers to the DASS-21 as the current mental health monitoring questionnaire, however this has since been replaced with the COMPI Fertility Problem Stress scale, COMPI coping strategy scale, and Fertility Quality of Life Questionnaire. These questionnaires are fertility specific and will help our counsellors provide more specifically targeted assistance if needed.
You will need to complete and return this questionnaire prior to beginning a treatment cycle. All questions must be answered.
